Head Start Enrollment

About Enrolling in Head Start

Our program offers year-round enrollment for children who have turned three or four years old before July 1st of the program year. All applicants must meet Federal guidelines as they relate to income. Please review the enrollment application or contact your county office for more information about availability.

Head Start Enrollment Form

Please fill out the form below to begin your child's enrollment in the Head Start program. Review these important details before you proceed:

Application Process & What to Expect

  • Application Submission: All applications are stamped upon receipt by the Office of Elementary Education. They are then placed in preschool on a first-come, first-served basis and undergo a selection process.
  • Next Steps: Once your application is received and processed, Family Service Workers will contact you to set up an enrollment appointment.

Documents Required at Enrollment Appointment

Please ensure you have the following documents ready for your child's actual enrollment appointment:

  • Up-to-date shot record
  • Birth certificate from the Department of Vital Statistics in the state where the child was born (hospital or county birth certificates are NOT accepted).
  • Current WV Health Check form from the child’s health care provider
  • Certification of a dental exam within the last 12 months
  • Proof of income for Head Start eligibility.
